Recently bought an early production 1991A1, used, in a Colt Blue Plastic box. Box is smaller than the ones used on later 1911's and has no label; tho I'm certain its original to the gun. Question: did some of these come with a cardboard cover, somewhat like the ones that cover the styrofoam inserts?

Yes, they did. The outer case was a cardboard slip case covered in sepia-like reproductions of older Colt print advertisements.

Scott: much obliged for the feedback. Wish they had stayed with that type box made specific for for 1911's. Not so big and gawky. :)
