Any ideas on this: the spent shell stays in the barrel almost every other shot, but will rapid fire no problems. 1911 commander size/9mm . thanks

Am I understanding this? Shooting slowfire cause a FTE every couple rounds, but rapid fire is no prob?

I would make sure that your extractor is tensioned correctly, and that your firing pin stop in also fitted properly, to stop the extractor from pivoting and possibly not grabbing the case at the correct angle.
