This is not uncommon with magazines that have a flat follower. Try some Wilson 7-rounders (or mags with a similar domed follower) and you'll probably never experience this again.
I actually had an Armscor that did this all the time with a variety of magazines. Turned out the extractor tunnel was drilled too high in the slide so the extractor sat higher than it should have.
