Been shooting IPSC Production for a few months now and could not actually see the holes when I hit the target especially from a distance like 12 yard or more.
I think I am too eager or rushing to move to the next target. But sometimes even if I take more time I can't still see the holes.

Any suggestions or if there is a good thing to practice to improve will be appreciated.

Your sight picture will tell you where the shot went. You should not be wasting your time looking at the target. A suggestion would be to practice calling your shots by setting up a plain bullseye target at 25 yds., fire a shot, call it's location by the sight picture you had when the gun went bang, then look to see where the shot really went. After a while you will be able to accurately call the shot placement. This is an old Bullseye training technique, but it applies to all shooting.
