Similar thing happened to a friend some 20 years ago. We were at Davis indoor gun range. We noticed that something fell on the floor in front of the shooting benches. My friend started making smart ass comments about another shooters gun, "what a piece of junk". My comment was "hey John that came off your gun..."
I am told that you should check the screw on the ejector rod assembly to make sure its tight. I use the low strength purple loctite on my single actions.
I was told by the range master that it happens occasionally.
