That sounds to me as if you have a holster problem and not a firearm one....I have discovered the safety had slipped off into fire position on each of my 4 Sig P938/P238s while the pistol was firmly in its holster on my waist. This is always discovered at the end of the day when removing the pistol from the holster...

Now I know why you are having problems. Do not mess with any loaded handgun on your person to do any safety BS check as that is only monkeying with the gun and you will have, as you have discovered, problems. Buy a good holster and insert your CCW and then leave it alone until:...The ability to check safe status without removing the pistol from my holster is a huge advantage...
Are you fellas by chance using cheapo nylon Uncle Mike type holsters? Safeties (in working order) don't just mysteriously change positions unless something physically moved them. This happens on all three of my Kimber Micros as well.
I appendix carry. I don’t like having the hammer dig into my gut so the SFS system offers a comfortable solution to this issue.
